If you child requires medicine to be administered during the school day, please complete this form and pass the medicine to the class teacher or school office. 

PARENTAL AGREEMENT FOR SCHOOL TO ADMINISTER MEDICINE: 
The school will not give your child medicine unless you complete and sign this form. Under no circumstances will medicine be accepted if any of these conditions are not met. 

We can only administer medicine prescribed by a doctor and in its original container. 

Please complete 1 form per child.
