Medication

If your child needs medicine during the school day, please complete the form below and hand the medication to the class teacher or school office.

Parental consent for school to administer medicine
School staff can only give medication if this form has been completed and signed.
Medication will not be accepted or administered unless all requirements are met.

• We can only administer medicine prescribed by a doctor

• Medication must be in its original container with the prescription label attached

• One form must be completed per child
